I had the exact same problem and it turned out to be the strut was out of alignment with the stern tube, resulting in an amazing amount of lash once I put in a new cutless bearing.
Let's hope it's not that since it was a huge PITA and stupidly expensive to fix. (Drop rudder, cut shaft because coupling is bonded on and it was warped anyhow, remove strut from hull and rebed in perfect alignment, new shaft, new coupling, new PSS shaft seal because the whipping destroyed the old one, etc.).
