It was for that reason that I hesitated getting a wooden plaque made. How
do I attach it permanently? And fashionable? And 4" tall?

I did include the "NO." on both my epoxy attempts.

I was boarded by the Coast Guard and when asked for ID my wife and I each
presented our OUPV 6-pack licenses and requisite TWIC cards. They looked
at the documents funny and asked if we just had a driver's license. When
asked for a rules of the road manual, we retrieved the one that I had
purchased 4 years ago. They again looked at it funny, thumbed the pages
and stated, "That's newer than any copy we have. It's always nice when we
pull over sailboaters, they always have their shit together."
