I put a windlass on Secret Plans 7 years ago and have loved it ever since - makes anchoring so much easier that it isn't thought of as a hassle. I glassed a beam into the back of the anchor locker, fastened an aluminum bracket to it, and attached to the windlass to that - so it is not above deck at all.
I'm running rope plus 65 ft chain and a 15kg Rocna. Other than the cost and effort, another negative for a windlasses is the possibility for corrosion an break down. Hauling the 15kg Bruce + 10m of heavy chain is do-able but not ideal especially in deep, windy, tight spaces.
