It would be nice if there existed ACCURATE installation/configuration docs on getting Apache to work properly with Cocoon 2.0 and Tomcat 4.0.1 For example mod_jserv.c is NOT supplied with the source to Tomcat 4.0.1 (the last I saw it was around 3.2.3). If you go to the source download directories you find webapp-module instead, so presumably this the preferred Apache module to use.
