You have probably already done this, but it bit me once so I thought I'd share what happened to me. Make sure the entire directory path from / all the way down is readable and executable. I once had the same problem, and it turned out that a directory way up in the hierarchy was not set to be readable by the cocoon user.

Probably too simple, but had to share.
