The big change that's needed is to give up on all metadata being in RVM.
If we can store metadata in regular files, then coda could scale with
the underlying filesystem.  This is probably a huge change, but if coda
is going to remain (or become, really) relevant for real use then I
think this is going to have to happen sooner or later.
