On line 439 of Email.pm, try modifying it to be:

my $smtp = Net::SMTP->new($Codestriker::mailhost);

to

my $smtp = Net::SMTP->new($Codestriker::mailhost, Debug => 1);

Also - check the stderr from apache's error log file - there might be
some useful information there too.
