[phone] The CallScreen hid itself before dialing completed
Due us not creating CallScreenEntry's if we have no valid
modemidentifier we would the CallScreen from updateAll. Stop
doing that by checking if we are currently dialing and only
if not hide the callscreen.
Tested with incoming and outgoing calls in the phonesim

[callscreen] Do not assume that on emit listEmpty the callscreen is hidden
Hide the secondary callscreen only when the main callscreen gets hidden
as well. The previous code assumed that PhoneLauncher would hide the
CallScreen so it hid the secondary after it... do it properly
