The skill families table was out of date. This PR brings it in line
with what's actually in `.claude/skills/`:
- **security**: 8 → 9 skills (adds `security-issue-sync`)
- **issue**: new family row — 5 skills covering triage, reproduction,
fix drafting, and backlog re-assessment
- **mentoring**: new row for the proposed family; links to
`docs/mentoring/README.md` and notes the `pr-management-mentor`
prototype skill
- **utilities**: new row for the two meta-skills (`write-skill`,
`list-steward-skills`)
- Updates the intro sentence to reflect four shipping families plus
one proposed
