Thanks for the detailed report and for linking the related discussion. One small clarification from the author of #74761: that PR is still open and has not been merged. Its scope is to propagate DBAPIError and StaleDataError to the existing transaction retry boundary; it does not address all of the isolation concerns in #59120. It also leaves CommitProhibitorGuard unchanged and introduces no SAVEPOINTs, so it does not cover the DynamicRegion.get_or_create_many() path reported here. The nested-transaction interaction you describe looks relevant to the earlier discussion, although I have not independently reproduced this particular case yet.
