#576: Realm/id decoration not working in TracReports

If ticket ID column is selected in a report then generated links lack
`href` attribute. There are other instances of this same bug related in
general to realm/ID decoration in report results.
Detected while running the functional test suite. Test case =
`trac.ticket.tests.functional.TestReportRealmDecoration`
