At least on [https://trac-hacks.org/wiki/AccountManagerPlugin?action=edit
trac-hacks] pages, we tend to put the !PageOutline first, then the title
heading. In general, we probably want to avoid adding constraints on the
order of content in a wiki page.

Add padding around `trac-content`, such as wiki page content. The padding
is 19px, which is the same as the Bootstrap `well` class. Refs #306.
