Thanks for finding and clearly explaining this bug. It has been fixed directly in master by https://github.com/apache/casbin/commit/a7718555c28cfa92e4c22c7ddd90c739b56f7af2, which uses the same approach (resolve every index before writing, then remove the old keys and insert the new ones) and also checks that oldRules and newRules have the same length.
