Updated most of the suggested changes, I wasn't able to simplify StreamHeader,
StreamContext and PendingFile.
I've run various test for Request & Transfer and seems good, however there was
one case when I got an error flushing the SSTable, looks like compaction
happened before the file transfer could be completed, on a request. I'm trying
to reproduce that, in the meantime here's the patch with updates.
