This might be due to mmap'ed index file access - Windows has some known issues 
with deleting files that have actively mmap'ed segments live in memory (see 
CASSANDRA-6993).  Given your data file deleted correctly and access violation 
hit on index, this is the first thing that jumps out to me.  If you see the 
following in your system.log:"DiskAccessMode 'auto' determined to be mmap, 
indexAccessMode is mmap", adding the following to your conf\cassandra.yaml file 
might fix this:
disk_access_mode: standard

Please let me know if the above addresses this and I'll push for the 6993 
backport to make it into the next 2.X release.
