Similar to the patch for CASSANDRA-1527, this validates the creation of
ColumnDefs from YAML and then from thrift/avro.
For simplicity, both name and index need to be set or unset at the same time. I
don't think anyone is complaining that it's too hard to introduce non-obvious
bugs, so I'm being explicit here.
