It would also be useful to be able to do:
INSERT INTO foo(x, y, z) SELECT a, b, c FROM bar;
That is, you already have a table set up and want to INSERT into it. This is
sort of under the covers of CTAS (step 1: create the table; step 2: insert the
data into it).
