I finally found a 
[dtest|https://github.com/stef1927/cassandra-dtest/commits/10231] that can 
reproduce this pretty reliably. The trick was to crash and restart a node 
several times until the DN entry with null host id appears, at which point if 
we no longer restart the node the entry gets stuck.
