Alright, I've rebased again on the latest from 9921 in another [new
branch|https://github.com/thobbs/cassandra/tree/9664-rebase-2]. The problem
with {{ALTER TABLE}} still exists, so there are still a couple of commented out
unit tests. However, this should be good enough for [~slebresne] to start code
review.
