Fix For: 2.1.x, 2.2.x
{{SSTableWriter.appendFromStream}} calls {{ColumnIndex.build}} only after
finishing the row and writing an {{END_OF_ROW}} marker. After CASSANDRA-7953,
the latter may still need to write tombstones.
