After reverting to batching by replica rather than just by partition key, see 
discussion on CASSANDRA-9303, the problem _almost_ disappears. 

I think there is some other issue, it happened twice all day long and I ran 
these bulk import tests pretty much all day fixing other issues for 
CASSANDRA-9303. In both cases the Cassandra processes lock the CPU, both times 
on 2.2 HEAD despite running several tests on TRUNK as well. 

I'm attaching some log files.

twice out of approximately 50 times. Both failures were on 2.2 HEAD and never 
on TRUNK despite running about half of the tests on TRUNK.
