The failure was caused by the recently added
[{{UpdatingClusterMetadataWrapper}}|https://github.com/stef1927/cassandra-dtest/blob/master/cqlsh_tests/cqlsh_copy_tests.py#L334].
The driver logs the exception that caused the metadata refresh to fail but
only at DEBUG level, so if trying to reproduce we must make sure to capture the
driver debug log messages.
