It seems likely that this is the same problem as [CASSANDRA-11884]. Current
thoughts is that the failures are caused by [CASSANDRA-9669], and Branimir is
investigating. Your call, but I'd probably either resolve this as duplicate for
now or at least wait to see how that developers.
