I've rebased the branch on top of the [CASSANDRA-12374], [CASSANDRA-12223] and
[CASSANDRA-12378] and removed {{TokenTreeSerializationHelper}} and pretty much
everything that's not necessary for the issue at hand.
I've added upgrade tests and dtests
