I would propose to simply rename the param "isClReplay" in Keyspace.apply() to 
"mayThrowWTE" or "mayTimeout" or sth similar with default true. IMHO 
"isClReplay" is bad style as the flag should indicate a semantic, not a caller 
source.

Keyspace.applyFromCommitLog just has to set writeCommitLog=false + mayTimeout = 
false
Introduce Keyspace.applyFromStream and set writeCommitLog=true + mayTimeout = 
false

In StreamReceiveTask, the Keyspace could be opened once per SSTableReader, then 
the Keyspace wouldn't have to be opened again for every mutation. Like this:
