In documentation -> CQL -> Data definition  -> The clustering columns

CREATE TABLE t (
    a int,
    b int,
    c int,
    PRIMARY KEY (a, c, d) ----> this is wrong
);

should be 
CREATE TABLE t (
    a int,
    b int,
    c int,
    PRIMARY KEY (a, b, c)
);
