Jeremy Hanna resolved CASSANDRA-2426.

Resolution: Fixed
I checked the server logs and it was evident that the cluster thought there was
a node down somewhere so it couldn't come to agreement. I had removed that
token though, so I think this might be another manifestation of CASSANDRA-2371.
