Some initial tests with a couple CCM clusters pointed out two rather glaring
issues that needed to be addressed:
1.) The V5 throw-on-overload case was not properly releasing bytes in flight,
and the tests that verified this didn't catch it, because they floored the
queue capacity too late to take effect.
2.) The V4 backpressure logic didn't properly consume rate limiter permits
after breaching a bytes in flight limit.
Both of these have been fixed

I've also collected data from a full battery of single node tests, but the
results are almost exactly the same as those above, so I probably won't post
them again. Now that single nodes are behaving the way they should, I'll be
moving (for real this time) back to the multi-node tests...
