Sorry about the delay to this, my free time this month has been pretty sparse.

This patch combines all previous changes and uses the new hint storage format 
we discussed above.

On my machine, a pile of unrelated tests are failing (mostly in 
SSTable.estimateRowsFromIndex), but I suspect this is an issue with my test 
setup.
