My first pass at review is complete, and I've left my comments in the new PR. I
imagine we'll have some back and forth at this point, primarily around how much
we can simplify the packed balanced tree code/format. Also, I'll try to do a
double-check before we merge to make sure my comments on the old PR were
addressed, etc.
