Also a patch that removes refcounting (and omits the copy above) to check what 
the performance hit is over pre-CASSANDRA-2521 sstable deletions.

Note that this is NOT intended as a "real" patch; it does not actually add back 
in GC-based sstable deletion.  No sstables will be removed w/o a server 
restart, but that should be adequate for testing read performance.
