Glad to see we worked 1) and 3) out.
For 2), I suspect that's a bug on the html2 generator. I will take a look at
their [project|https://github.com/OpenAPITools/openapi-generator] issues
backlog and raise an issue if it's not there (they have around 4.5K issues
reported, so I do think someone must have seen this before).
For reference, I'm also updating the example html that current PR code generates
