wrt #1, yes the warnings and rejections does not cover every scenario and does
not provide any guarantees, and yes can be erroneous too. So you get
availability reduced with rejections without any guarantees provided. Treat
both warnings and rejections as a warning system and attempt to reduce the
amount of misplaced writes, but still assume that misplaced writes have
happened and take suitable steps to remedy/restore.
