Fix {{RequestFailureReason}} and {{ReadCommand}} serializers and address some
minor inconsistencies in a few other serializers.
{{RequestFailureReason}} serializer incorrectly uses {{computeVIntSize()}}
instead of {{computeUnsignedVIntSize()} in {{serializedSize()}.
{{ReadCommand}} incorrectly uses the wrong epoch field to calculate
{{serializedSize()}}.
