Description: As mentioned on CASSANDRA-4127, for upgrades we need a
'shuffle' command to split up the contiguous ranges. (was: As mentioned on

move without a token, in the cases where entropy is not your friend and gives
you an unbalanced cluster (I've seen up to a 7% discrepancy myself))
