### Summary
This PR adds support for GitHub App token refresh. It is based on issues we
faced due to token expiration with large projects. The initial idea was to fix
the token refresh issue only, but once that was working other issues appeared -
e.g., the connections request from the UI to the GitHub plugin returned
incorrect/corrupt token values which also had to be fixed.
Claude Opus 4.6 was used during the work.
