### What problem does this PR solve?

Problem Summary: Light schema changes can expose newly added nested fields
through DefaultValueColumnIterator on old segments. Nested-column pruning
splits predicate and lazy-output reads into two phases, but the default-value
iterator ignored the phase contract and appended values in both phases,
producing inconsistent child sizes. Make both sequential and rowid reads use
the existing placeholder/recovery lifecycle so only selected rows are
materialized after filtering.
