### What problem does this PR solve?
Related PR: #69087
Backport #69087 to `branch-4.2` (source commit
`01e735b29c8b8803b9e2df10b46b9b02db1e6251`).
- Prevent Segment TopN from inserting an incomplete condition-cache entry
after reaching its local read limit, which can cause later queries to miss rows.
- Allow COUNT_ON_INDEX with residual predicates that reference no columns,
such as `random() < 2`, while continuing to reject residual predicates that
reference columns.
- Include both regression suites and their expected output files from the
original PR.
The include conflict in `olap_scan_operator.cpp` was resolved by adding
`<set>` to the 4.2 includes. All added and removed lines match the original
patch.
### Validation
- clang-format 16 check passed for both changed C++ files.
- Verified that the added and removed lines match the original PR and that
all four regression files are byte-identical.
- Source whitespace check passed; generated `.out` files retain their
expected trailing blank lines.
- Build and regression tests were not run for this backport.
### Release note
