Fixed in 00e856e49e7. Automatic missing-table repair now clears connector,
derived metadata and FE object/ID state at database scope in both case-folding
modes, while retaining the exact observation and metadata-refresh-generation
guards. Added Foo -> FOO -> Foo tests for modes 1 and 2: both fail before the
fix and pass afterward, and the final lookup loads the third incarnation schema
instead of reusing historical state.
