   ### What problem does this PR solve?

Independent Code Review Runner jobs can select the same Codex auth object
and run concurrently. Select an eligible auth before starting the worker, then
queue the worker in a GitHub concurrency group for that auth. The slot remains
exclusive through review execution, capacity retries, and credential writeback.
The runtime canary shares the production auth slot, and existing per-PR
serialization is retained.

The worker reloads and validates its assigned credential after waiting. If
that auth has been removed, cooled down, or quarantined, the job fails without
switching to an unlocked auth. Re-run **all jobs** to select another auth;
rerunning failed jobs alone can retain the original assignment. Auth discovery
remains dynamic, but queued requests do not migrate between slots.

This change bounds concurrency; it does not claim that serialization
eliminates model capacity errors. Model selection, retry limits, and review
subagents are unchanged. Older workflow runs must finish before validating the
new per-auth exclusivity during rollout.

   ### Release note

   ### Check List (For Author)

   - Test
       - [x] Unit Test: Offline Linux workflow suite: **340 passed, 2 opt-in
real gh HTTP tests skipped**. Includes ten new regressions covering auth
assignment, credential changes while queued, unavailable-slot failures, canary
lock sharing, and PR queue admission.
       - [x] Manual test: YAML parsing, all 30 inline Bash steps and the shared
auth shell syntax, and `git diff --check` passed. Actionlint 1.7.12 passed with
only its unsupported `concurrency.queue` diagnostic excluded; `queue: max` is
documented by GitHub and already used on the base branch. Live GitHub queue
behavior remains to be validated after rollout.
   - Behavior changed:
       - [x] Yes. Reviews sharing an auth wait in its queue. There is no
automatic reassignment when a queued auth becomes unavailable.
   - Does this need documentation?
       - [x] No. Internal CI behavior; ownership and lifecycle are documented
in workflow comments, and failure messages explain full-workflow reruns.
