kernel - Improve cache_fullpath(), plus cleanup
* Improve cache_fullpath(). It can use a shared lock rather than an
exclusive lock, significantly improving concurrency. Important now
since realpath() indirectly uses this function.
* Code cleanup. Remove unused vfscache_rollup_all()
