Thank you for submitting a pull request! Here are some tips:

   1. For first-time contributors, please read our contributing guide:
      https://github.com/apache/gluten/blob/main/CONTRIBUTING.md
   2. If necessary, create a GitHub issue for discussion beforehand to avoid
duplicate work.
   3. If the PR is specific to a single backend, include [VL] or [CH] in the PR
title to indicate the
      Velox or ClickHouse backend, respectively.
   4. If the PR is not ready for review, please mark it as a draft.

   ## What changes are proposed in this pull request?

   Fixes #13402

Fix incorrect VELOX labeling for Bolt-only changes and add Bolt support
   to the issue templates.

   - Exclude `cpp/bolt/**/*` from the VELOX C++ rule.
   - Add BOLT matching for `backends-bolt/**/*` and C++ files outside
     `cpp/velox/**/*`.
   - Use `all-globs-to-any-file` so C++ inclusion and exclusion conditions
apply to the same changed file.
   - Add BOLT (Bolt) to the Bug Report backend dropdown.
   - Update the Build problem prompt to ask which Gluten backend was built,
how it was built, and what issue occurred.

Shared C++ changes and PRs touching both backends receive both labels.
   Other label rules remain unchanged.

   ## How was this patch tested?

   - Parsed all 35 tracked YAML files and inspected selectable options.
   - Verified the Bug Report backend options and required-field setting.
   - Verified the Build problem prompt is backend-neutral.
   - Eight local path-matching simulations passed, covering Bolt-only,
     Velox-only, mixed-backend, shared C++, documentation, and Substrait
changes.
   - License-header checks passed for all three changed files.
   - `git diff --check` passed.

Validation was local. The GitHub Actions workflow and rendered issue
forms have not been tested on GitHub. No runtime code was changed.

   ## Was this patch authored or co-authored using generative AI tooling?

   Generated-by: OpenAI Codex (GPT-6)
