   ## Summary

994c9d8929d ("sched:use tcb inside of pthread_tcb_s, remove all cast")
   renamed `struct pthread_tcb_s` to `struct pthread_entry_s` and dropped the
embedded `struct tcb_s` member, but left the comment block above the
structure untouched. It still named `struct pthread_tcb_s` and explained
why tasks and pthreads had separate TCB forms, which after that commit was
false in both respects: pthreads allocate a common `struct tcb_s` and append
   a `struct pthread_entry_s` for their startup data
   (`kmm_zalloc(sizeof(struct tcb_s) + sizeof(struct pthread_entry_s))` in
   `sched/pthread/pthread_create.c`), fetching it with
   `(FAR struct pthread_entry_s *)(tcb + 1)`.

This PR replaces the header and that paragraph with a description of the
current layout, so the file no longer advertises a type that does not exist
and a memory-saving rationale that does not apply. The text matches the
   code in 994c9d8929d.

   ## Impact

Comment only (`include/nuttx/sched.h`). No functional change.

   ## Testing

   `tools/checkpatch.sh -f include/nuttx/sched.h` passes.
