fix couchdb boot: ulimit -n 1048576 + readiness probe
Wrap couchdb entrypoint with ulimit -n 1048576 to avoid erl_child_setup
spinning on 2^31 fds when running under docker daemons that default to
unlimited nofile (systemd default, WSL2 native docker, etc.). Add tcp
readiness probe on 5984 so the pod doesn't advertise as ready before
CouchDB actually accepts connections.
