When a segment upload fails, PinotSinkWriter.flush() throws (correct), but the failed future stays in _pendingUploads because waitForPendingUploads() throws before clear(). During task cleanup Flink calls close(), which calls flush() again and rethrows the same exception. Flink logs "FATAL - exception in exception handler of task" and shuts down the TaskExecutor. Reproducible with an auth-enabled controller and no authToken (upload gets 401), see #20121.
