Review Comment:
   What do you think of letting modules bring their own exception mapper? I 
feel like that gives us the best of both words: good dependency hygiene and 
extensibility as new modules introduce new exceptions.
   
   If you think that could work, I'll file an issue and then maybe merge this 
as-is. Otherwise, I can just add this dependency now too.
