In the https://github.com/apache/pulsar/compare/v3.3.1...v3.3.2 diff, #23072 looks like the largest change related to acknowledgements. If you would be able to run the experiments mentioned in https://github.com/apache/pulsar/issues/23908#issuecomment-2626717978, that would confirm many things. Building a fork off v3.3.2 with #23072 commit 1996c86 reverted would be one way to check if it's impacting your use case.
