I took a stab at a solution for parts #2 and #3. The attached patch
(rol-1518.patch) duplicates the submit buttons in the global and individual
comments forms, placing a copy at the top. In addition, it adds a "Per page"
drop-down menu. The menu is populated with options for 20, 40, 60, 80 and 100.
With 20 being the default. I have also adding a small bit of date logic so that
a user selecting the same start and end dates will see all the results that
fall between 00:00:00 and 23:59:59 of the selected date.
