You do need to file an ICLA with Apache. The form is here:
http://www.apache.org/licenses/icla.txt
You do not need a proposal, but it's usually a good idea to discuss your plans
on the mailing list. You might get some good feedback and that could greatly
improve the chances of your patch being accepted.
Are you working against the 4.0 code base or the latest from trunk?
