We resolved the matter of the disabled account creation, I have another JIRA 
already to update the documentation so others don't get tripped up in the same 
manner.

As for prepopulating the email and username fields from LDAP data, looking at 
the code it appears it would be quite difficult and time-consuming to do and 
test properly and I can't see anyone going to put in that amount of effort just 
to save users from a few seconds of typing.  But in the future if someone wants 
to create a patch for it and open up a new JIRA we'll consider it.  It would 
need to be something though that can work for multiple LDAP servers, if it 
causes things to break we'd have to take it out again.
